This policy establishes clear procedures for managing incidents to protect participants, staff, and stakeholders. We are committed to creating a safe environment through proactive risk management, timely incident response, and continuous improvement based on lessons learned.

WellVibe is committed to maintaining the highest standards of safety and care. We ensure all incidents are reported, investigated, and managed in accordance with NDIS Practice Standards and relevant legislation, with a focus on participant wellbeing and continuous quality improvement.
Types of Incidents We Manage
Critical Incidents
Death, serious injury, abuse, neglect, or unlawful sexual or physical contact
Reportable Incidents
Incidents requiring notification to the NDIS Commission within 24 hours
General Incidents
Minor injuries, near misses, property damage, or service delivery issues
Incident Management Process
Immediate Response
ImmediateEnsure safety of all involved, provide first aid if needed, secure the scene, and contact emergency services if required.
Incident Reporting
Within 24 hoursDocument the incident using our incident report form, including all relevant details, witnesses, and immediate actions taken.
Notification
Within 24 hoursNotify relevant parties including the NDIS Commission (for reportable incidents), participant representatives, and management.
Investigation
Within 5 daysConduct thorough investigation to identify root causes, contributing factors, and gather all relevant information.
Action Plan
Within 10 daysDevelop and implement corrective actions to prevent recurrence, including policy updates, training, or procedural changes.
Review & Learning
OngoingReview incident outcomes, share learnings across the organization, and update risk management strategies.
NDIS Commission Reporting Requirements
Reportable Incidents (24 hours)
- Death of a participant
- Serious injury requiring medical treatment
- Abuse or neglect of a participant
- Unlawful sexual or physical contact
- Sexual misconduct
- Unauthorised use of restrictive practices
